What this app is for
Build a workplace risk assessment on your phone — activity, site, hazards with likelihood × severity, controls, leftover (residual) risk, assessor signature — then share a PDF. Works offline. No account required for core use.
Suggested flow
- New assessment → fill the header (activity, site, date, prepared by).
- Hazards → add rows (chips help), set L/S, controls, then leftover after controls.
- Review → fix blockers; confirm if leftover risk stays High or Extreme.
- Sign & share → sign, preview the same layout as the final PDF, then Share PDF.
Leftover risk
Each hazard asks what remains after controls (lowered / same / stop work). High or Extreme leftover shows a warning on review and on the PDF so you do not silently clear a serious residual.
Free vs Unlock
Free: a limited number of first-time final PDF shares per calendar month (shown on Home). Re-sharing a completed assessment does not use a free slot. Copy as new then Share counts as a new first-time share.
Unlock (one-time purchase, not a subscription): unlimited first-time shares. When free slots are used up, New assessment prompts Unlock.
Local price is shown in Google Play / the App Store before purchase. New phone? Use Restore purchases with the same store account.
Extra signature (supervisor / worker)
In Settings → Supervisor / worker signature:
- Off — assessor signature only.
- On — second pad is shown but optional; you can still Share without it.
- Required — second signature must be captured before Share PDF.
Backup (no cloud)
Assessments stay on the device only. Before changing phones, use Settings → Export assessments — the JSON includes hazard text and signature images. Keep that file private. Import adds copies; it does not overwrite Unlock. This is not live cloud sync.
Opening a completed assessment is view-only (locked). Use Share again (free). To change content, go Home → Copy as new.
Likelihood (L) and Severity (S) use a 1–5 scale. The selected word (e.g. Possible / Major) appears next to the label; Risk is L × S.
Send feedback (one-way)
Settings → Send feedback. We do not reply in the app; we read when we can. Do not include passwords, worker PII you should not share, or other people’s sensitive data.
